FOR LOCAL FIGHTERS


1) First secure a Games and Amusements Board (GAB) license similar to
the ordinary professional boxer with or without a manager. For you to
be able to do this, you must have the budget of at least P2,000.00
because this will cover the entire process including the medical
tests, licensing fee and notary of forms. The location of the GAB
office is at the 3rd floor of Legaspi Tower 200, Paseo De Roxas,
Legaspi Village (near 7 Eleven) in Makati City.


2) Partial Breakdown of Expenses:

1X1 ID photos (4 pcs.) / 2X2 ID photos (8 pcs.) = P100.00 at Kodak
for just 5 minutes
Residence Certificate (Cedula) = P10.00 just go to your local
municipal or city hall
Drug Test = P150.00 (at Angelus Clinic, but may take test at any
other DOH accredited clinic)
Hepatitis Test = P150.00 (at Angelus Clinic, but may take test at any
other DOH accredited clinic)
ECG Test = P150.00 (at Angelus Clinic, but may take test at any other
DOH accredited clinic)
X-Ray = P150.00 (at Angelus Clinic, but may take test at any other
DOH accredited clinic)
GAB license fee = P600.00
GAB license official notary = P70.00

3) Now the next thing you have to do is go to the boxing office of
GAB and ask for a blue form for a "Fighter/Unarmed Combatant" license
application. You'll have to bring this form along with you until the
whole process is finished. Then go to the medical office where you
will find the doctor giving you a referral form to "Angelus Medical
Clinic" for the medical tests and check-up. Make sure you do all of
this during regular office hours from 9AM to 5PM only, not on
weekends and during their lunch break time from 12-1pm. NOTE: Make
sure you get the phone number of GAB medical office for future
reference – checking up on results, etc.

(You may choose to take your Medical Tests at any DOH accredited
clinic/hospital rather than Angelus Medical Clinic. If this is the
option you choose, please make sure to find out from the GAB medical
office exactly what tests you need from the clinic. Also, make sure
to keep the results of the tests if you decide to have your medical
tests somewhere else rather than Angeles Medical Clinic. With the
results in your hands, you can give the results to the GAB Medical
Office and skip to step number 6 below.)


4) The location of Angelus clinic is walking distance from GAB
office. Ask the employees for the exact address and directions. On
the medical referral GAB provides you, there should also be a map
behind it to Angelus Clinic. Upon exiting the building, take a left
and at the very first corner make another left. At the fifth corner
you will have to go left again and locate the "Medical Tower
Building". If you see the building, Angelus clinic is just in front
of it. Ask the guard for assistance, they will be a BIG help. Now
upon entering the building, you will be designated to different rooms
for different tests. First you have to go to a doctor where she or he
will bill you for the expenses. Secondly, go to the cashier section
and pay for the exams. Now you have to go to the X-ray room and get
the test. After that, the ECG, then the Drug test where they have to
get some blood out of you and urine test.


5) Now you'll have to wait for a day or two and Angelus clinic will
deliver the results to the GAB medical office. Contact GAB medical
office (don't forget to get their number) for the confirmation of the
arrival of your results.


6) Upon confirmation of your medical results, go back to GAB medical
office and request for the doctor's approval of your form. Then go to
the boxing division office where you must get your form signed by the
official who will check your "cedula" or residence certificate. Then
this person will bill you for the licensing fee where you have to pay
it at the cashier. After all this you will go back to the person who
billed you and he or she will ask for the payment of the official
notary. Now, she will ask you for your right thumb mark and sign the
License ID. Now your are finished. You will get the ID within a month
or two but the "receipt" is as good as the license so hold on to it.


7) Contact the FEARLESS MMA Championship tournament director at 0918-
921-9802 or the Matchmaker at matchmaker@..., to let
him know that you have obtained a Professional Fighter/Unarmed
Combatant license from GAB and that you are interested to compete.
Personnel from FEARLESS will then coordinate with you personally as
to what steps to take next.
