Membership Renewal Form
PLEASE DISREGARD THE 2005 CDS DUES ENVELOPE
YOU RECEIVED IN THE MAIL

An error was made (by the printer) on the membership renewal envelope form. The 2005 membership envelope with the old dues were sent out.

You will be receiving a corrected membership renewal envelope in the mail this week. Please disregard the first mailer and use the correct form with the correct dues amount. $65 Active and $55 Additional Family.

If you already mailed in $60, please send in an additional $5.00 to complete your membership for 2006.
