Please excuse the glaringly obvious edits. It has been some years
since I've done this form and I made several mistakes. I did not have
the time to go back and redo the form, so I had to edit the mistakes out.
I tried to keep a slow and steady pace for the form. I hope my
movements are clear. I will answer any questions you may have if any
particular movement is too hard to make out.
