We get our girls to fill out medical consent form, this has all their
personal and medical history on it. eg. ambulance cover, private health
cover, medical conditions such as diabetes, asthma and what course of
action to take if they have a attack.
This information is kept by the secretary and brought to every game day.
We had a instance where one of our girls was knocked out, we called a
ambulance and were able to give all the information that they needed name
address medicare no. ambulance and private health insurance any allergies.
