Joel,
It really does sound like you need help with your stroke (most likely you are not breathing properly but that can be cause by any number of stroke issues).
You need to find a swim team that can help you and that has a coach. This way you can afford a coach as the entire team pays for the coaching.
I am the President of Nepean Masters Swim Club so I am biased to which team I think is the best. We have a link on our club site that links to all the local clubs.
Take your time, find an affordable club that meets your time schedule and then register and swim a few times a week. You should communicate to the club your skill level and then you can ask if the coach works on strokes.

At our club we have 2 coaches per session (each coach coaches four lanes with one coach focusing on the newer swimmers and one coach focusing on the more experienced swimmers - except our Thursday evening and Saturday morning swims which have only one coach).
