I'm far from an expert, but I won't let that stop me from giving advice!
In my case, when I have a flair up, it seems to ache earlier and
earlier each long run. I found that the throbbing/aching is unpleasant
but not enough to stop me. When I ended up with shooting pains, those
stopped me cold.
If you want to have no throbbing during your race, then you'll want to
be more conservative now. As you know from your previous IT
experience, it's not going to get better without an intervention. It
seems that one of the best interventions is not running!
Have you been rolling, icing, stretching, etc.? You might also
consider intervals as that doesn't seem to aggravate it as much.
