Ok everyone we have a CMYM Bay t-shirt design and are
ready for the team order. See the attached file for
the logo design and placement. We need to get this
done BEFORE the bay so see the dates below...

T-shirt prices are:
$10.00 per shirt for short sleeves
$15.00 per shirt for long sleeves

White, 100% Cotton
2-color Design

T-shirt orders including payment:
Due by May 21st, 2004
Cash or Check (make payable to Howard County Ymca)
Please use a separate order form for each
individual's order.
Late orders will NOT be accepted!!!
No extra shirts are being ordered for this event.

Return Order form and Payment to Coach Mike Jacobson
by May 21st! Shirts will be available for pick-up
during the 1st week of June.

Sizes are S,M,L,XL,XXL in short or long sleeves.
