It's too late to mail out the form if you still want to play. So please send out
an email to indicate which events you want to play asap. So the organizers will
have a clear idea of the number of participants in this event.

Also, every participant will get a *free massage* compliments of Re.Vita Health
Center.

Location: Prairie Stone Sports & Wellness Center, 5050 Sedge Blvd, Hoffman
Estates, IL 60192. (About 40 min North-West from Chicago)
Eligibility: Open to all level players
Events: Men's single, Men's double, Women's single, Women's double and
Mixed double (MS, MD, WS, WD, XD).
Format: Double Elimination. New score system. Everyone play at least
two games.
Entry Fee: $20 for the first event, $10 for each additional event.
Junior under 18 years old, $15 for the first event, $5 for extra event
Note: No Junior division only price is different.

Schedule: 11/04/06
8:30am Registration for Men's Single and Women's single.
Games start at 9:00am sharply.
10:00am Mix double
12:30pm Men's double and Women's double
7:30pm all games finished
