Hello Chicago Rush Group,

My name is David White--I’m a writer for West Suburban Living magazine
working on an article about the Chicago Rush. The magazine is a sort of
interesting-and-fun-things-to-do-while-living-in-Chicagoland type of
magazine, so the article will be a general one--history of the Rush, when
they play, what the games are like, what fun and exciting things happen at
the games, etc.

I am looking for some feedback from Rush fans and was wondering if any (or
all) of you might be willing to take a few minutes to fill out the following
questionnaire. If so, you may be quoted in the article.

So--if you have time over the next few days, I’d appreciate any thoughts you
might like to share about the team. The article is for the March/April
issue of the magazine, which is available throughout the western suburbs.
