Tom,
You could have shifting problems. Also, increased wear on the chain
because of side loading. Did the frame come to you this way?
if you obtained from BentTech I would talk with them about this
alignment problem, 10 mm offset seems too high to me. On my first
frame I'm about 2-3 mm offset.

On your bolt problem, are the treads bad or is the lug misaligned? I
used bolt through the lugs so I wouldn't have a problem with treads in
the lug. Your idea of a longer bolt with a nut is what I use and have
no problem
