WDIA Teams,
I have attached the 2009 WDIA roster verification form which all WDIA teams are required to complete to maintain their eligibility within the WDIA. Please complete the attached form and
MAIL the original copy to me directly by February 15th. Please be advised that your school's Registrar needs to certify on the document that all players listed are full-time students as defined by your University.
All team forms from our league must go to US Lacrosse in one package by the final March 1st deadline. Please mail the original to me at the following address:

Please keep in mind the following:

It takes time to have your Registrar sign off on the form so start the process now.

Teams who fail to complete the attached roster will NOT be eligible to attend Nationals.

Players not certified as full-time students on the attached roster will NOT be allowed to play at Nationals.

Should our league not have the required minimum number of teams (8) remain WDIA eligible, our league loses its Automatic Qualifier to Nationals for a period of 2 years.
