This is not one of the two that you're looking for, but I was the catcher in a
little league game in '69 or '70 in which there were three, catcher-to-third
(25) CS in one inning. Coincidentally (or maybe not), my brother was the third
baseman. Unfortunately, there is no published historical record of this feat.
