I have a dvd at home that shows John McSherry emphatically calling a batter out on strike 2. I don't think it mentions the year. With the semester ending and finals looming, I'm not likely to get a chance to try to find out the pitcher or batter any time soon.

Subject: [SABR_Records] 3-ball
walks and 2-strike strikeouts
In Monday's Cardinals/Brewers game, Albert Pujols went
to first base on
a walk after having received only three balls. No one noticed
(including the umpire) that he still needed 1 more ball to get a walk
per the rule book. I've seen a few plays like this over the years,
including one or two cases of a batter getting called out on 2
strikes...but I can't remember the details of any of them.
Does anyone happen to have any specifics of other recent cases where a
player walked on less than 4 balls, or struck out on less than 3
strikes?
Thanks,
Trent McCotter
