How common is it these days for a double-header not to be 'separate
admission?' I can't remember hearing of one, although I don't track
double-headers in any detail.

A few years back (sorry for not remembering the year), when Montreal played some games in Puerto Rico, one of the PR games against the Giants was rained out...
