REMINDERS: Must have at least 5 USA Judo registered members under the club that
is being registered. One (1) of them must be the person listed "Certified
Coach"/"Certified Teacher" as the Instructor/Coach of record. This individual
must have undergone or must undergo a background screening (done through USA
Judo, USJA, or USJF). All background checks are valid for 4 years.
