I was trying to get the attachment put on the FJI website but the server is not
letting me log in right now (and I am leaving town for Colorado Springs, CO for
the USA Judo meetings and US Open).
So those that have selected "Individual e-mails" will be able to get the
attachment (this one is in MS Word format). Otherwise contact Brian Germain or
myself via e-mail (when I get back) and one of us can get you the attachment.
