Since Blaine found the guidelines for bike lane:
(5) Drainage Grates and Manhole
Covers
Locate drainage inlet grates and manhole covers
to avoid bike lanes. When drainage grates or
manhole covers are located in a bike lane,
minimize the effect on bicyclists.
A minimum of
3 feet of lateral clearance is needed between the
edge of a drainage inlet grate and the shoulder
stripe. Install and maintain grates and manhole
covers level with the surface of the bike lane.

In West Olympia in the area that they redid at Division St NW and Bowman Ave NW cyclist have an issue with a rain grate that sits about 1  foot out from the corner of the curb.  If you are going from Bowman to Division toward the college you have to swing out into the flow of traffic on Division before entering the bike lane to keep your wheel from dropping into the grate.  The grate posses a similar issue for going north on Division but to a lesser degree.  If you are making the the slits in the grate line up perfectly to catch a road bike wheel.  If simply riding on Division it could catch your wheel if you simply turn your wheel just a bit while crossing it.  Since they can not dig up the road and do it again I think the issue could be bettered if they simply pulled the rate and rotated it 90 degrees.  Since the curb is not close enough to channel water anyway it sould not make any differance for flood contol which way it faces.
