Hi everyone, I'm in the process of getting our member roster together and need to collect everone's info.  If you could please send me the following information, in the following format it would be great

 

First Name, Last Name, age, Address, phone number, USA Triathlong Number (if you have one, if not just leave a space), emergency contact name and relation, emergency contact phone number

If you wan't to tell me anything else interesting about yourself feel free.
