Ray,
I agree with Jeff, these things are hard to daignose worlds apart.
But... let's give it a long shot. I guessing, that maybe, you can
back out the *set screw* on your derailleur (by a hair), to allow
a little slack for your grip shifter to reach the *click*, (index).
***Make sure, you don't back it out too much, that your chain would
fall in between the gear & spokes.

It's tough to tell the problem from this distance. The indexing mechanism
is indeed in the grip, so it might have gotten gummed up somehow. I'd also
be looking at the cable and housing to make sure there's no kinks or
frayed spots.
If it's been in regular use for more than a year or two, it would probably
be good to replace the cable and housing, simply as preventative
maintainence. I would also disassemble the shifter and clean and regrease
it, just to make sure everything is working properly. It's a simple job,
but if you're not familiar with bikes it can be tricky. Here's the dealer

FWIW: I've had variable success with SRAM's cheaper parts. The older
shifters were terrible, but the new ones seem to have held up. YMMV.
